8/19/2023 EnchantaVaughn Approved Behavior: Client was able to go straight to therapeutic area in preparation for her session. Client displayed non-aggressive behaviors and was seen following all directions and listening to QMHP and mother, indicating that she was already having a good time in session. Client was observed sitting quietly at her seat reviewing therapeutic assignment along with her counselor. Client seemed to be in a positive mood, as evidenced by her happy expressions and excitement to come to family session. Client demonstrated ability to multi task by working independently and staying on task. Intervention: QMHP facilitated a discussion to explore client feelings and things she liked and disliked about counseling at home. QMHP offered active listening skills and showed client respect while she was sharing her ideas and thoughts about services. QMHP praised client for her cooperation and managing her behaviors as best as she could. QMHP used appropriate behavior modeling by pointing out a peer that is demonstrating positive behavior to decrease the client’s impulsive behavior and encourage her to use self-control. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). Response: Client wants to make her counselor and parent proud by doing a good job at home. Client is very independent but can be a team player as well. Client reviewed last week discussion with psycho educational and retained a lot of the information she was taught which motivates her to pay more attention and take her time. Client is observed problem solving on her own and handling her emotions appropriately. Client avoids meltdowns and lashing out when upset or frustrated. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence.
